We collaborate with patient flow & bed management teams to offer a gradual and structured pathway for individuals, ensuring they receive the right care, attention, and guidance at each step of their recovery process.

We offer short term placements for delayed transfer of care, patients set to be discharged from hospital settings and offer support with sourcing long term accommodation or care.

We’ll work with housing providers, community mental health teams, home-based treatment teams as well as other care providers to ensure a safe and timely transition.
